Please call our office at 301-694-0900 to schedule an appointment with Dr. Bakker, Dr. Mostofi or Sarah Jamieson, PA-C.
Our office hours are:
Monday, Tuesday, Thursday, Friday: 8:00 am to 5:00 pm
Wednesday: 8:00 am to 6:30 pm (extended hours in Frederick only)
Last Saturday of the Month: 8:00 am to 5:00 pm (Frederick only)
Our office staff will be happy to find an appointment that fits your needs. You will receive an appointment reminder by phone 3 days prior to your scheduled appointment that also serves as a reminder to request any needed insurance referrals prior to your appointment in our office.
To prepare for your appointment, please bring:
- Completed new patient paperwork (if applicable)
- Insurance card(s)
- Any required insurance referrals
- Photo ID
- A list of your previous medical providers, especially if they are neurologists or sleep specialists, and arrange for records from those providers to be sent to our office prior to your appointment
- Test results such as MRIs, CT scans, lab work, EEGs and sleep studies
- A written list of all medications you are currently taking, including strengths and dosages
- A diary of your symptoms and a list of questions that you would like to address with the provider
