Getting Started

The first step is to schedule an initial consultation with one of our providers. Please have your insurance cards and your referral with you when you call for your appointment. If you have been referred for testing only, please also have your physician’s order handy when calling. To help make your first visit with us go as smoothly as possible, our courteous scheduling staff will collect all of your demographic information during your initial phone call, and our highly trained billing staff will verify your insurance coverage for you prior to your visit.

During your initial phone call, you may also be given a username and password to access our patient portal.  We encourage you to learn more about the conveniences the patient portal will provide you.

Your first consultation will last approximately 30-60 minutes, during which time the doctor or physician assistant will talk with you about your medical history and will perform a complete neurological examination. The provider will discuss with you their findings and impressions, and will outline a plan for treatment and/or possible further testing. Appointments for testing can last anywhere from 30-90 minutes depending on what testing your provider has requested. Our scheduling staff will help you understand what to expect during your visit.

To provide you with the best possible care and a more comprehensive initial consultation, we strongly encourage you to bring the following items with you to your appointment:

  • Completed new patient packet (can be downloaded here or mailed to you if requested):
  • New Patient Packet
  • Any medical and surgical records related to your current problem
  • A diary of your symptoms
  • A list of your current medications with strengths and dosages
  • Actual films of any MRIs or CT scans, if applicable to your current problem
  • Your insurance card(s)
  • Insurance referral from your primary care doctor, as required.

Become a New Patient

We are currently accepting new patients into our practice. Thank you for considering us. To become a new patient:

  1. Sign up for our patient portal
  2. Download your patient forms online through the patient portal

When you come to our office for the first time as a new patient, we'll ask you to complete some initial forms, including an Authorization and Consent for Treatment form, if you were not able to download them from the patient portal in advance of your appointment.

To make sure there are no delays in care during your first visit experience, please arrive 15 minutes prior to your scheduled appointment to ensure your registration is complete before meeting with your new provider.

Remember to bring:

  • Your insurance card
  • Valid photo ID
  • List of current medications
  • Office co-pay

In an effort to respect the time of all of patients, our staff strives to stay on schedule so that other patients do not have to wait.

For patients who are delayed and arrive late for appointment, every effort will be made to see them the same day. However, wait times may apply, or appointments may need to be rescheduled.

Patient Forms

Authorization for Release of Medical Information (PDF) - Allows patients to authorize the disclosure of their health information to a designated individual, company, agency, or facility.

Preferred Contacts (PDF) - Patients are encouraged to complete and return the Preferred Contacts Form but it is not required. Contactos Preferidos
